The Swing-Bed Facility is a fourteen (14) bed short-term specialty program dedicated to the provision of skilled nursing and rehabilitative care to patients who need additional time to transition from the hospital and return to the community.

Admission Criteria:

• Three (3) day qualifying hospital stay (Medicare); within the past 30 days.
  
• Medically stable, in accordance with hospital standards.
  
• Must have an identifiable skilled need related to the reason for hospitalization.
  
• Must be screened by Discharge Planning to determine eligibility for admission.
  
• Must require inpatient care.
  
• Must require skilled services on a daily basis.
